Patient Information

If you have scheduled an initial visit with us for treatment, it will be necessary to fill out a Patient Information and a Health History form for our records.  They can be obtained by clicking on the links below.  Please download the forms and save them to your hard drive. Complete the forms before your first visit if possible, and bring them with you to save time and allow us to proceed with your treatment more quickly.
